1. GENERAL
CITY WILL OBTAIN AN ENCROACHMENT PERMIT FROM THE COUNTY OF SAN BERNARDINO
TRANSPORTATION DEPARTMENT (FOR WORK TO BE PERFORMED WITHIN COUNTY
RIGHT-OF-WAY).
CONTRACTOR SHALL MAINTAIN TWO LANES OF TRAFFIC AT ALL TIMES DURING
CONSTRUCTION UNLESS OTHERWISE SPECIFIED OR PERMITTED BY THE CITY. CONTRACTOR
SHALL FURNISH TRENCH PROTECTION AS REQUIRED TO CONSTRUCT SEWER AND MAINTAIN
TRAFFIC MOVEMENT.
2. NOTIFICATIONS
ONE MONTH PRIOR TO BEGINNING CONSTRUCTION, CONTRACTOR SHALL POST
CONSTRUCTION NOTICE SIGNS SPECIFYING DATES WHEN CONSTRUCTION WILL BEGIN AND
CONTRACTOR'S NAME AND PHONE NUMBER.
IN ADDITION TO NOTIFICATIONS SPECIFIED IN THE CONSTRUCTION SPECIFICATIONS
(GENERAL CONDITIONS, UTILITY REQUIREMENTS), CONTRACTOR SHALL NOTIFY RESIDENTS
AND COMMERCIAL ESTABLISHMENTS OF IMPENDING WORK. PRINTED NOTICES, PROVIDED
BY CITY, SHALL BE DELIVERED TO ALL RESIDENTS AND COMMERCIAL ESTABLISHMENTS IN
THE VICINITY OF THE WORK AT LEAST SEVEN DAYS IN ADVANCE OF CONSTRUCTION.
SAID NOTICES SHALL FIRST BE APPROVED BY THE CITY AND SHALL CONTAIN A GENERAL
DESCRIPTION OF THE WORK, DATES WORK WILL BE PERFORMED, DESCRIPTIONS OF AREAS
WHERE TRAVEL AND PARKING WILL BE RESTRICTED, AND NAMES OF STREETS WHICH WILL
BE CLOSED TO THROUGH TRAFFIC OR WHERE TRAFFIC WILL BE RESTRICTED.
IF THE CITY OR COUNTY RECEIVES COMPLAINTS FROM INDIVIDUALS AFFECTED BY
CONSTRUCTION, CONTRACTOR SHALL TAKE IMMEDIATE ACTION TO CORRECT THE
SITUATION AS DIRECTED BY THE CITY. IF CONTRACTOR RECEIVES COMPLAINTS DIRECTLY,
HE SHALL REPORT SAME IMMEDIATELY TO THE CITY. THEREAFTER, HE SHALL TAKE
IMMEDIATE ACTION TO CORRECT THE SITUATION AS DIRECTED BY THE CITY.
3. MANHOLE GRADE ADJUSTMENT
CONTRACTOR SHALL INSTALL NEW MANHOLE SHAFTS TO FINISH GRADE AND PLACE
MANHOLE FRAME AND COVER.
4. BACKFILL
WHERE PROPOSED SEWER CROSS BENEATH EXISTING WATERLINES (4" MINIMUM
DIAMETER), CONTRACTOR SHALL WRAP EXISTING WATERLINE WITH 60 ml VISQUINE AND
CRADDLE EXISTING WATERLINE WITH 2" THICK STYROFOAM TO THE SPRINGLINE.
CONTRACTOR SHALL PROVIDE A PIPE SUPPORT FOR THE EXISTING WATERLINE PER
DETAIL BELOW.

5. SEWER LATERALS
APPROXIMATE LOCATIONS OF LATERALS ARE SPECIFIED ON THE CONSTRUCTION
DRAWINGS. CONTRACTOR SHALL COORDINATE WITH CITY AND EACH PROPERTY OWNER TO
DETERMINE PRECISE LATERAL LOCATIONS. THEREAFTER, CONTRACTOR SHALL CONSTRUCT
LATERAL AT THE DETERMINED LOCATION. THE SEWER LATERALS ARE TO BE
CONSTRUCTED FROM PROPOSED SEWER TO EXISTING RIGHT-OF-WAY OR TO 1' BEHIND
CURB AS SPECIFIED ON THE CONSTRUCTION DRAWINGS.
IN ACCORDANCE WITH STATE DEPARTMENT OF HEALTH SERVICES REQUIREMENTS,
LATERALS SHALL BE CONSTRUCTED A MINIMUM OF 10' FROM EXISTING PARALLEL WATER
FACILITIES.
6. RESTORATION REQUIREMENTS
CONTRACTOR SHALL PROTECT EXISTING CURB, GUTTER, AND SIDEWALK IN PLACE DURING
SEWER CONSTRUCTION. IF CURB, GUTTER, AND/OR SIDEWALK IS DAMAGED DURING
CONSTRUCTION, CONTRACTOR SHALL REPLACE SAME TO LIMITS SPECIFIED BY CITY.
CONTRACTOR SHALL, AS A MINIMUM, HAVE ACCOMPLISHED BY THE END OF EACH WORK
DAY (4:00 P.M.) THE FOLLOWING:
A. PLATE OPEN EXCAVATIONS (30' MAXIMUM). TEMPORARY ASPHALT CONCRETE SHALL
BE PLACED AROUND PLATES FOR SMOOTH TRANSITION (WHERE APPLICABLE).
B. FILL ALL REMAINING EXCAVATIONS WITH COMPACTED BACKFILL AND PLACE 2"
MINIMUM TEMPORARY ASPHALT CONCRETE PAVEMENT IN ALL AREAS WHERE EXISTING
PAVEMENT WAS REMOVED AND AT ALL DRIVEWAYS. SAID PAVEMENT SHALL BE
COMPACTED, ROLLED SMOOTH AND FLUSH WITH ADJACENT PAVEMENT SUFFICIENT TO
SUPPORT TRAFFIC. SAID PAVEMENT MAY BE PLACED COLD PROVIDED IT IS PLACED
SMOOTH AND FLUSH WITH ADJACENT EXISTING PAVEMENT AND ROLLED WITH A STEEL
WHEELED PAVEMENT ROLLER, AND PROPERLY MAINTAINED.
C. REMOVE ALL DEBRIS, CONSTRUCTION MATERIALS, AND EQUIPMENT FROM PUBLIC
STREETS, CONSTRUCTION RIGHT-OF-WAY, AND PRIVATE PROPERTY.
D. CLEAN (INCLUDING SWEEPING AND WASHING) ALL STREETS, DRIVEWAYS, GUTTERS,
SIDEWALKS, AND ALL OTHER WORK AREAS. (CONTRACTOR MAY BE REQUIRED TO WASH
AND/OR SWEEP WORK AREAS MORE THAN ONCE A DAY, IF DIRECTED BY THE CITY).
E. RELOCATE ALL TRAFFIC CONTROL AS REQUIRED AND TO OPEN ALL STREETS TO
TRAFFIC.
CONTRACTOR SHALL MAINTAIN ALL TEMPORARY ASPHALT CONCRETE THROUGHOUT
THE COURSE OF CONSTRUCTION. CONTRACTOR SHALL BE RESPONSIBLE AT ALL
TIMES TO MAINTAIN TEMPORARY ASPHALT CONCRETE AND TRENCH BACKFILL IN
GOOD AND SAFE CONDITION. CONTRACTOR SHALL IMMEDIATELY RESPOND TO ALL
REQUESTS BY CITY FOR REMEDIAL WORK (INCLUDING WEEKENDS AND HOLIDAYS).
CONTRACTOR SHALL HAVE A CREW AVAILABLE TO RESPOND TO SAID REQUESTS.
CONTRACTOR SHALL RECYCLE ALL REMOVED ASPHALT CONCRETE PAVEMENT.
CONTRACTOR SHALL FURNISH CITY WRITTEN DOCUMENTATION THAT SAID PAVEMENT
HAS BEEN RECYCLED.
7. ASPHALT CONCRETE PAVEMENT/STRIPING/SIGNING
FOR ALL AREAS WHERE PROPOSED ASPHALT CONCRETE MEETS EXISTING ASPHALT
CONCRETE, SAW CUT EDGES TO NEAT VERTICAL EDGE EITHER PERPENDICULAR TO OR
PARALLEL WITH CENTERLINE. CONTRACTOR SHALL PROVIDE ALL STRIPING AS SHOWN ON
THE TRAFFIC CONTROL PLANS AND AS NECESSARY TO RELOCATE TRAFFIC. CONTRACTOR
SHALL FURNISH TRAFFIC SIGNING AS REQUIRED INCLUDING BUT NOT LIMITED TO, BUS
STOP SIGNS, SPEED LIMIT SIGNS, AND CONSTRUCTION SIGNS.
8. SEWER CONSTRUCTION SEQUENCE
CONTRACTOR SHALL CONSTRUCT SEWERS CONTINUOUSLY FROM DOWNSTREAM TO
UPSTREAM STATION.
9. TRENCHING
CONTRACTOR SHALL WHILE ACTIVELY WORKING IN CONSTRUCTION AREA, LIMIT OPEN
TRENCHES TO 100 FEET MAXIMUM. NO OPEN TRENCHES ARE PERMITTED DURING NON
WORK HOURS.
NOTES
1. WIDTH OF BEAM SHALL EQUAL O.D. + 8" OF SUPPORTED PIPE.
2. IF SUPPORTED PIPE IS BEDDED IN CONCRETE, BEAM WIDTH SHALL
BE EQUAL BEDDING WIDTH.
3. IF BEAM IS PRECAST, ENDS OF BEAM SHALL BE FULLY BEDDED IN
420-C-2000 CONCRETE FOR LENGTH 18". THE BEDDING SHALL HAVE
A MINIMUM THICKNESS OF 4". CLASS'C' MORTAR SHALL BE PLACED
BETWEEN TOP OF BEAM AND SUPPORTED PIPE TO PROVIDE
MINIMUM BEARING SHOWN.
4. THIS CASE IS PERMITTED ONLY IF THE TRENCH WALLS ARE FIRM
AND UNYIELDING.
5. MAXIMUM SPACING OF BARS SHALL BE 4" O.C.

GENERAL NOTES:
1. ALL WORK SHALL BE IN ACCORDANCE WITH THESE PLANS, THE CITY OF
FONTANA STANDARD PLANS, THE CONTRACT PROVISIONS AND THE STANDARD
SPECIFICATIONS FOR PUBLIC WORKS CONSTRUCTION ("GREEN BOOK"). ALL
REFERENCE SPECIFICATIONS AND STANDARDS SHALL BE THE LATEST EDITION

2. CONSTRUCTION PERMITS SHALL BE OBTAINED FROM THE CITY OF FONTANA

START OF ANY WORK. INSPECTION COORDINATION SHALL BE REQUESTED AT
LEAST TWO WORKING DAYS PRIOR TO THE START OF ANY WORK IN PUBLIC

3. THE CONTRACTOR SHALL NOT OPERATE ANY FIRE HYDRANT OR WATER MAIN
VALVES. CONTRACTOR SHALL COORDINATE WITH THE APPROPRIATE WATER
COMPANY FOR VALVE OPERATION AND WATER REQUIREMENTS.
4. STATIONING REFERS TO THE CENTERLINE OF STREET EXCEPT WHERE
OTHERWISE NOTED.
5. ALL UNDERGROUND UTILITIES SHALL BE INSTALLED, TESTED AND APPROVED
PRIOR TO PAVING OF STREETS.
6. THE LOCATIONS OF UTILITIES SHOWN HAVE BEEN DETERMINED FROM
AVAILABLE INFORMATION. HOWEVER, IT SHALL BE THE RESPONSIBILITY OF THE
CONTRACTOR TO DETERMINE, IN THE FIELD, THE TRUE LOCATION AND ELEVATION
OF ANY EXISTING UTILITIES, AND TO EXERCISE PROPER PRECAUTION TO AVOID
DAMAGE THERETO. THE CONTRACTOR SHALL CONTACT UNDERGROUND SERVICE
ALERT AT 1-800-227-2600 TWO WORKING DAYS BEFORE EXCAVATION.
7. ALL MANHOLES, CLEAN OUT FRAMES, COVERS AND VALVE BOXES SHALL BE
RAISED TO FINISHED GRADE BY THE PAVING CONTRACTOR UPON COMPLETION OF
PAVING.
8. THE CONTRACTOR SHALL COORDINATE CONSTRUCTION WITH ALL UTILITY
COMPANIES INCLUDING, BUT NOT LIMITED TO, GAS, TELEPHONE, ELECTRIC, CABLE
TELEVISION, LANDSCAPING, LANDSCAPE IRRIGATION, DOMESTIC WATER, RECLAIMED
WATER, SEWER, STORM DRAIN, FLOOD CONTROL AND CALTRANS. ALL UTILITY
COMPANIES SHALL BE GIVEN TWO WORKING DAYS NOTICE PRIOR TO WORK
AROUND THEIR FACILITIES.
9. AS -BUILT DRAWINGS SHALL BE PROVIDED BY THE CONTRACTOR TO THE
ENGINEER OF RECORD, WHO SHALL PROVIDE RECORD DRAWINGS TO THE CITY
ENGINEER.
10. WHEN CONFLICT IS FOUND TO EXIST IN THE CONTRACT DOCUMENTS THAT
CAN NOT BE RESOLVED BY REFERENCE TO PRECEDENCE PROVISIONS IN THE
"GREEN BOOK", THE CONTRACTOR SHALL IMMEDIATELY REPORT SAID CONFLICT
TO THE CITY ENGINEER FOR RESOLUTION.
11. APPROVED SOIL STERILANT IS REQUIRED UNDER ALL NEW PERMANENT
ASPHALT PAVEMENT PRIOR TO PLACEMENT.
12. ALL EXPOSED CONCRETE SURFACES SHALL CONFORM IN GRADE, COLOR AND
FINISH TO MATCH EXISTING CONCRETE.
13. NO CONCRETE SHALL BE PLACED UNTIL THE FORMS AND REINFORCING
STEEL HAVE BEEN PLACED, INSPECTED AND APPROVED.
14. THE WALLS AND FACES OF ALL EXCAVATIONS GREATER THAN FIVE (5) FEET
IN DEPTH SHALL BE GUARDED BY SHORING, SLOPING OF THE GROUND OR
OTHER APPROVED MEANS PURSUANT TO THE REQUIREMENTS OF THE DIVISION
OF INDUSTRIAL SAFETY OF THE STATE OF CALIFORNIA. TRENCHES LESS THAN
FIVE (5) FEET SHALL ALSO BE GUARDED WHEN THE POTENTIAL EXISTS FOR
GROUND MOVEMENT.
15. THE CONTRACTOR SHALL OBTAIN A PERMIT TO PERFORM EXCAVATION OR
TRENCH WORK FOR TRENCHES 5 FEET OR GREATER IN DEPTH FROM THE
CALIFORNIA STATE DIVISION OF INDUSTRIAL SAFETY.
16. NO TRENCH BACKFILL SHALL TAKE PLACE WITHOUT PRIOR APPROVAL OF
THE CITY INSPECTOR.
17. THE CONTRACTOR SHALL NOT CAUSE ANY EXCAVATED MATERIAL, MUD SILT
OR DEBRIS TO BE DEPOSITED ONTO PUBLIC OR PRIVATE PROPERTY ADJACENT
TO THE RIGHT OF WAY DURING CONSTRUCTION WITHOUT PRIOR WRITTEN
APPROVAL.
18. IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO MAINTAIN
BARRICADES, DELINEATORS OR OTHER TRAFFIC CONTROL AT ALL TIMES.
19. A GEOTECHNICAL ENGINEER SHALL CERTIFY ALL BACKFILL COMPACTION.
FAILURE TO OBTAIN THE REQUIRED DENSITY SHALL REQUIRE REWORKING OF
THAT PORTION OF THE WORK UNTIL THE SPECIFIED DENSITY IS OBTAINED.
20. ALL MATERIALS AND METHODS ARE SUBJECT TO THE APPROVAL OF THE
CITY ENGINEER.
21. ADEQUATE CONSTRUCTION CONTROL STAKES SHALL BE SET BY THE
ENGINEER TO ENABLE THE CONTRACTOR TO CONSTRUCT THE WORK TO PLAN
GRADES. THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE PRESERVATION OF
BENCHMARKS AND CONSTRUCTION CONTROL STAKING DURING CONSTRUCTION.
22. CARE SHOULD BE TAKEN TO PREVENT GRADES, DITCHES, AND SWALES
FROM UNDERMINING STREET IMPROVEMENTS. UPON INSPECTION OF THE SITE, THE
CITY ENGINEER MAY REQUIRE TEMPORARY NON-ERODEABLE SWALES ENTERING
OR LEAVING IMPROVEMENTS.
23. IF EXISTING UTILITIES OR ANY OTHER FACILITIES CONFLICT WITH THE
PROPOSED IMPROVEMENTS, THE CONTRACTOR SHALL NOTIFY THE ENGINEER AND
ALL AFFECTED AGENCIES IMMEDIATELY.
24. THE CONTRACTOR SHALL NOT DISTURB EXISTING SURVEY MONUMENTS,
MONUMENT TIES OR BENCH MARKS WITHOUT PRIOR NOTIFICATION TO THE CITY
ENGINEER.
25. REMOVAL AND REPLACEMENT OF EXISTING SURVEY CONTROL, INCLUDING
SURVEY MONUMENTS, MONUMENT TIES AND BENCH MARKS, SHALL BE DONE BY
A REGISTERED CIVIL ENGINEER OR LICENSED LAND SURVEYOR. SURVEY
MONUMENTS THAT WILL BE DESTROYED AS A RESULT OF THIS CONSTRUCTION
SHALL BE REPLACED. THE CONTRACTOR SHALL NOTIFY THE ENGINEER ONE WEEK
PRIOR TO CONSTRUCTION SO THAT TIES TO MONUMENTS CAN BE ESTABLISHED
FOR LATER REPLACEMENT OF THE MONUMENT.
26. THE CONTRACTOR SHALL MAINTAIN ACCESS FOR LOCAL RESIDENTS AND
BUSINESSES AT ALL TIMES. A MINIMUM TWELVE FOOT LANE SHALL BE
MAINTAINED AT ALL TIMES IN THE CLOSURE AREA FOR RESIDENTS AND
EMERGENCY VEHICLES.

SEWER IMPROVEMENTS
FOR
LOCUST AVENUE
27. NO MATERIAL OR EQUIPMENT SHALL BE STORED IN THE PUBLIC RIGHT OF WAY
WITHOUT OBTAINING A SEPARATE PERMIT FOR THAT PURPOSE.
28. SANITARY SEWER LINES SHALL BE CONSTRUCTED OF BELL AND SPIGOT OR
BAND SEAL TYPE VITRIFIED CLAY PIPE (V.C.P.), EXTRA STRENGTH CLASS AND SHALL
CONFORM TO THE PROVISIONS OF THE ASTM DESIGNATION C700 FOR V. C. P.
29. THE SEWER CONTRACTOR SHALL STAMP AN "S" IN THE FACE OF THE CURB AT
THE LOCATION OF THE SEWER LATERAL.
30. IF SEWER CONSTRUCTION IS TO BE DONE IN PHASES, THE CONTRACTOR SHALL
PROVIDE A TERMINAL CLEAN OUT AT THE PHASE BOUNDARY, NOT MORE THAN 150
FEET FROM THE DOWNSTREAM MANHOLE. SEE CITY OF FONTANA STANDARD DETAIL
FOR TERMINAL CLEAN OUT.
31. ALL HOUSE LATERALS SHALL BE INSTALLED AT A 2% MINIMUM GRADE UNLESS
OTHERWISE APPROVED BY THE CITY ENGINEER.
32. PRIOR TO CONSTRUCTION, THE CONTRACTOR SHALL EXPOSE EXISTING FACILITIES,
AND VERIFY ELEVATION AND LOCATION OF CONNECTIONS. CITY APPROVAL OF
CONNECTIONS TO EXISTING FACILITIES DOES NOT IMPLY CORRECTNESS OF
ELEVATIONS OR LOCATIONS SHOWN ON THE PLANS.
33. THE CONTRACTOR SHALL PROVIDE AND MAINTAIN AN EFFECTIVE MEANS OF
DUST CONTROL, INCLUDING ADEQUATE WATERING, AT ALL TIMES.
34. SHORT V.C.P. STUBS WITH FLEXIBLE COMPRESSION JOINTS SHALL BE USED AT
MANHOLE WALLS TO ALLOW FOR MINOR DEFLECTIONS IN ALIGNMENT.
35. THE CONTRACTOR SHALL CONFORM TO ALL TRAFFIC CONTROL POLICIES,
METHODS AND PROCEDURES DESCRIBED IN THE "W.A.T.C.H. MANUAL", LATEST
EDITION UNLESS OTHERWISE DIRECTED BY THE CITY TRAFFIC ENGINEER.
36. UPON COMPLETION OF CONSTRUCTION, CONTRACTOR SHALL RESTORE ALL
SIGNING, STRIPING, BARRICADES, AND OTHER TRAFFIC CONTROL DEVICES TO THE
SATISFACTION OF THE CITY TRAFFIC ENGINEER.
37. NO OPEN TRENCH SHALL BE ALLOWED AT THE END OF THE DAY WITHOUT PRIOR
APPROVAL OF THE CITY ENGINEER.
