THE CONTRACT PROVISIONS AND THE STANDARD SPECIFICATIONS FOR PUBLIC WORKS CONSTRUCTION
("GREEN BOOK"). ALL REFERENCE SPECIFICATIONS AND STANDARDS SHALL BE THE LATEST EDITION
UNLESS OTHERWISE NOTED.
2. WHEN A TECHNICAL CONFLICT IS FOUND TO EXIST IN THE CONTRACT DOCUMENTS THAT CAN NOT BE
RESOLVED BY REFERENCE TO PRECEDENCE PROVISIONS IN THE "GREEN BOOK", THE CONTRACTOR
SHALL IMMEDIATELY REPORT SAID CONFLICT TO THE CITY ENGINEER FOR RESOLUTION.
3. ALL MATERIALS AND METHODS ARE SUBJECT TO THE APPROVAL OF THE CITY ENGINEER.
4. CONSTRUCTION PERMITS SHALL BE OBTAINED FROM THE CITY OF FONTANA COMMUNITY DEVELOPMENT
DEPARTMENT, ENGINEERING DIVISION PRIOR TO THE START OF ANY WORK. INSPECTION COORDINATION
SHALL BE REQUESTED AT LEAST TWO WORKING DAYS PRIOR TO THE START OF ANY WORK IN PUBLIC
RIGHT-OF-WAY WITHIN THE CITY LIMITS. CALL (909) 350-7610.
5. THE CONTRACTOR SHALL CONFORM TO ALL TRAFFIC CONTROL POLICIES, METHODS AND PROCEDURES
DESCRIBED IN THE STATE OF CALIFORNIA MANUAL OF TRAFFIC CONTROLS, LATEST NON -METRIC
EDITION UNLESS OTHERWISE DIRECTED BY THE CITY TRAFFIC ENGINEER.
6. IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO MAINTAIN BARRICADES, DELINEATORS OR
OTHER TRAFFIC CONTROL DEVICES AT ALL TIMES.
7. THE CONTRACTOR SHALL OBTAIN A PERMIT TO PERFORM EXCAVATION OR TRENCH WORK FOR
TRENCHES 5 FEET OR GREATER IN DEPTH FROM THE CALIFORNIA STATE DIVISION OF INDUSTRIAL
SAFETY.
8. NO MATERIAL OR EQUIPMENT SHALL BE STORED IN THE PUBLIC RIGHT OF WAY WITHOUT OBTAINING
PERMISSION FROM ENGINEER.
9. THE LOCATIONS OF UTILITIES SHOWN HAVE BEEN DETERMINED FROM AVAILABLE INFORMATION,
HOWEVER, IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO DETERMINE, IN THE FIELD, THE
TRUE LOCATION AND ELEVATION OF ANY EXISTING UTILITIES, AND TO EXERCISE PROPER PRECAUTION
TO AVOID DAMAGE THERETO. THE CONTRACTOR SHALL CONTACT UNDERGROUND SERVICE ALERT AT
811 TWO WORDING DAYS BEFORE EXCAVATION.
10. THE CONTRACTOR SHALL COORDINATE CONSTRUCTION WITH ALL UTILITY COMPANIES INCLUDING, BUT
NOT LIMITED TO, GAS, TELEPHONE, ELECTRIC, CABLE TELEVISION, LANDSCAPING, LANDSCAPE
IRRIGATION, DOMESTIC WATER, RECLAIMED WATER, SEWER, STORM DRAIN, FLOOD CONTROL AND
CALTRANS. ALL UTILITY COMPANIES SHALL BE GIVEN TWO WORKING DAYS NOTICE PRIOR TO WORK
AROUND THEIR FACILITIES.
11. THE CONTRACTOR SHALL NOT OPERATE ANY FIRE HYDRANT OR WATER MAIN VALVES WITHOUT
APPROPRIATE AGENCY AUTHORIZATION. CONTRACTOR SHALL COORDINATE WITH THE APPROPRIATE
WATER COMPANY FOR VALVE OPERATION AND WATER REQUIREMENTS.
12. UPON COMPLETION OF CONSTRUCTION, CONTRACTOR SHALL RESTORE ALL SIGNING, STRIPING,
BARRICADES, AND OTHER TRAFFIC CONTROL DEVICES TO THE SATISFACTION OF THE CITY TRAFFIC
ENGINEER.
13. CONTRACTOR SHALL RELOCATE AND/OR REPLACE LANDSCAPING, SPRINKLERS AND SIDEWALKS
AFFECTED BY THE CONSTRUCTION TO THE SATISFACTION OF THE CITY ENGINEER.
14. BEST MANAGEMENT PRACTICES FOR CONSTRUCTION ACTIVITIES (SWPPP)
A. ERODED SEDIMENTS AND OTHER POLLUTANTS MUST BE RETAINED ON SITE AND MAY NOT BE
TRANSPORTED FROM THE SITE VIA SWALES, AREA DRAIN OR NATURAL DRAINAGE COURSES.
B. STOCKPILES OF EARTH AND OTHER CONSTRUCTION RELATED MATERIALS MUST BE PROTECTED
FROM BEING TRANSPORTED FROM THE SITE BY THE FORCES OF WIND OR WATER.
C. FUELS, OILS, SOLVENTS AND OTHER TOXIC MATERIAL MUST BE STORED IN ACCORDANCE WITH
THEIR LISTING AND ARE NOT TO CONTAMINATE THE SOIL AND SURFACE WATERS, ALL APPROVED
STORAGE CONTAINERS ARE TO BE PROTECTED FROM WEATHER, SPILLS MUST BE CLEANED UP
IMMEDIATELY AND DISPOSED OF IN A PROPER MANNER. SPILLS MAY NOT BE WASHED INTO THE
DRAINAGE SYSTEM.
D. EXCESS OR WASTE CONCRETE MAY NOT BE WASHED IN THE PUBLIC RIGHT OF WAY OR ANY OTHER
DRAINAGE SYSTEM. PROVISIONS SHALL BE MADE TO DRAIN CONCRETE WASTES ON SITE UNTIL
THEY CAN BE DISPOSED OF AS SOLID WASTE.
E. TRASH AND CONSTRUCTION RELATED DEBRIS MUST BE DEPOSITED INTO COVERED RECEPTACLE TO
PREVENT CONTAMINATION OF RAINWATER AND DISPERSAL BY WIND.
F. DEPOSITION OF SEDIMENTS, SOILS OR OTHER MATERIAL BY VEHICULAR TRAFFIC LEAVING THE
PROJECT SITE MUST NOT BE ALLOWED. THE CONSTRUCTION ENTRANCE ROADWAYS MUST BE
STABILIZED SO AS TO INHIBIT SEDIMENTS FROM BEING DEPOSITED INTO THE PUBLIC RIGHT OF WAY,
ACCIDENTAL OR BY OTHER MEANS.
G. ANY SLOPE WITH DISTURBED SOILS OR DENUDED OF VEGETATION MUST BE STABILIZED SO AS TO
INHIBIT EROSION BY WIND AND WATER.
15. NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM (NPDES)
A. UNLESS NOTED OTHERWISE, ALL EXISTING SITE IMPROVEMENTS SHALL BE PROTECTED IN PLACE.
B. ALL EXISTING CONDITIONS ARE TO BE VERIFIED AT THE SITE PRIOR TO BIDDING.
C. CLEAN UP OF WORK AND SUPERVISION IS PART OF THE PROJECT.
D. CONSTRUCTION ACTIVITIES OF ONE ACRE OR MORE SHALL REQUIRE A GENERAL CONSTRUCTION
STORM WATER PERMIT. FOR MORE INFORMATION, YOU MAY CALL THE STATE WATER RESOURCES
CONTROL BOARD - DIVISION OF WATER QUALITY AT (916) 657-1146.
16. ADVANCE CONSTRUCTION SIGNING INDICATING DURATION OF PROJECT SHALL BE IN PLACE ONE WEEK
PRIOR TO IMPLEMENTING DETOURS.
17. AS -BUILT DRAWINGS SHALL BE PROVIDED BY THE CONTRACTOR TO THE ENGINEER OF RECORD, WHO
SHALL PROVIDE RECORD DRAWINGS TO THE CITY ENGINEER.

GENERAL NOTES FOR
STREET IMPROVEMEN
1. CURVE DATA REFERS TO THE FACE OF CURB.
2. STATIONING REFERS TO THE CENTERLINE OF STREETS EXCEPT WHERE OTHERWISE NOTED.
3. ADEQUATE CONSTRUCTION CONTROL STAKES SHALL BE SET BY THE ENGINEER TO ENABLE THE
CONTRACTOR TO CONSTRUCT THE WORK TO THE PLAN GRADES. THE CONTRACTOR SHALL BE
RESPONSIBLE FOR THE PRESERVATION OF BENCHMARKS AND CONSTRUCTION CONTROL STAKING
DURING CONSTRUCTION.
4. THE CONTRACTOR SHALL NOT DISTURB EXISTING SURVEY MONUMENTS, MONUMENT TIES OR BENCH
MARKS WITHOUT PRIOR NOTIFICATION TO THE CITY ENGINEER.
5. REMOVAL AND REPLACEMENT OF EXISTING SURVEY CONTROL, INCLUDING SURVEY MONUMENTS,
MONUMENT TIES AND BENCH MARKS, SHALL BE DONE BY A REGISTERED CIVIL ENGINEER OR
LICENSED LAND SURVEYOR. SURVEY MONUMENTS THAT WILL BE DESTROYED AS A RESULT OF THIS
CONSTRUCTION SHALL BE REPLACED. THE CONTRACTOR SHALL NOTIFY THE ENGINEER ONE WEEK
PRIOR TO CONSTRUCTION SO THAT TIES TO MONUMENTS CAN BE ESTABLISHED FOR LATER
REPLACEMENT OF THE MONUMENT.
6. THE CONTRACTOR SHALL MAINTAIN ACCESS FOR LOCAL RESIDENTS AND BUSINESSES AT ALL TIMES.
A MINIMUM 12 FOOT LANE SHALL BE MAINTAINED AT ALL TIMES IN THE CONSTRUCTION AREA FOR
RESIDENTS AND EMERGENCY VEHICLES.
7. THE CONTRACTOR SHALL PROVIDE AND MAINTAIN AN EFFECTIVE MEANS OF DUST CONTROL,
INCLUDING ADEQUATE WATERING, AT ALL TIMES.
8. ALL GRADING OPERATIONS SHALL BE DISCONTINUED WHEN SUSTAINED WIND VELOCITIES EXCEED 25
MILES PER HOUR.
9. THE CONTRACTOR SHALL NOT CAUSE ANY EXCAVATED MATERIAL, MUD, SILT OR DEBRIS TO BE
DEPOSITED ONTO PUBLIC OR PRIVATE PROPERTY ADJACENT TO THE RIGHT OF WAY DURING
CONSTRUCTION WITHOUT PRIOR WRITTEN APPROVAL.
10. A GEOTECHNICAL ENGINEER SHALL CERTIFY ALL BACKFILL COMPACTION. FAILURE TO OBTAIN THE
REQUIRED DENSITY SHALL REQUIRE RE -WORKING OF THAT PORTION OF THE WORK UNTIL THE
SPECIFIED DENSITY IS OBTAINED.
11. CARE SHOULD BE TAKEN TO PREVENT GRADES, DITCHES, AND SWALES FROM UNDERMINING STREET
IMPROVEMENTS. UPON INSPECTION OF THE SITE, THE CITY ENGINEER MAY REQUIRE TEMPORARY
NON-ERODEABLE SWALES ENTERING OR LEAVING IMPROVEMENTS.
12. THE FINAL LOCATION AND WIDTH OF DRIVEWAY APPROACH APRONS SHALL BE APPROVED AT THE
TIME OF CONSTRUCTION AND SHALL CONFORM TO THE CITY OF FONTANA STANDARD DETAILS.
13. ALL EXPOSED CONCRETE SURFACES SHALL CONFORM IN GRADE, COLOR AND FINISH TO MATCH
EXISTING CONCRETE.
14. NO CONCRETE SHALL BE PLACED UNTIL THE FORMS AND REINFORCING STEEL HAVE BEEN PLACED,
INSPECTED AND APPROVED.
15. ALL UNDERGROUND UTILITIES SHALL BE INSTALLED, TESTED AND APPROVED PRIOR TO PAVING OF
STREETS.
16. APPROVED SOIL STERILANT IS REQUIRED UNDER ALL NEW ASPHALT PAVEMENT PRIOR TO PLACEMENT.
17. PAVEMENT STRUCTURAL SECTIONS SHOWN ARE MINIMUM AND SUBJECT TO REVISION AND APPROVAL
OF THE CITY ENGINEER AS DETERMINED BY SOILS TESTS TAKEN AFTER COMPLETION OF ROUGH
GRADING.
18. ALL MANHOLES, CLEANOUT FRAMES, COVERS AND VALVE BOXES SHALL BE RAISED TO FINISHED
GRADE BY THE PAVING CONTRACTOR UPON COMPLETION OF PAVING.
1. THE WALLS AND FACES OF ALL EXCAVATIONS GREATER THAN FIVE (5) FEET IN DEPTH SHALL BE
GUARDED BY SHORING, SLOPING OF THE GROUND OR OTHER APPROVED MEANS PURSUANT TO THE
REQUIREMENTS OF THE DIVISION OF INDUSTRIAL SAFETY OF THE STATE OF CALIFORNIA . TRENCHES
LESS THAN FIVE (5) FEET SHALL ALSO BE GUARDED WHEN THE POTENTIAL EXISTS FOR GROUND
MOVEMENT.
2. STATIONING REFERS TO THE CENTERLINE OF STORM DRAIN EXCEPT WHERE OTHERWISE NOTED.
3. NO TRENCH BACKFILL SHALL TAKE PLACE WITHOUT PRIOR APPROVAL OF THE CITY INSPECTOR.

CITY ENGINEER.
5. PRIOR TO CONSTRUCTION, THE CONTRACTOR SHALL EXPOSE EXISTING FACILITIES, AND VERIFY
ELEVATION AND LOCATION OF CONNECTIONS. CITY APPROVAL OF CONNECTIONS TO EXISTING
FACILITIES DOES NOT IMPLY CORRECTNESS OF ELEVATIONS OR LOCATIONS SHOWN ON THE PLANS.
6. IF EXISTING UTILITIES OR ANY OTHER FACILITIES CONFLICT WITH THE PROPOSED IMPROVEMENTS, THE
CONTRACTOR SHALL NOTIFY THE ENGINEER AND ALL AFFECTED AGENCIES IMMEDIATELY.
7. MINIMUM D -LOAD FOR ALL RCP IS 1350.
8. ADEQUATE CONSTRUCTION CONTROL STAKES SHALL BE SET BY THE ENGINEER TO ENABLE THE

FOR STREETS AND HIGHWAYS, DATED SEPTEMBER 2006.
2. PAVEMENT MARKINGS SHALL CONFORM TO THE CITY- OF FONTANA STANDARD STENCILS.
3. ALL STRIPING AND PAVEMENT MARKINGS SHALL BE ALKYD THERMOPLASTIC PER SPECIAL PROVISIONS.
4. TRAFFIC SIGN SIZE SHALL BE FOR CONVENTIONAL ROADWAYS UNLESS OTHERWISE SHOWN ',ON THE
PLANS.
5. ALL SIGNS SHALL BE MOUNTED ON A GALVANIZED STEEL POST, UNLESS OTHERWISE SHOWN, PER
CITY STANDARD DETAIL NO. 4003.
6. IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO MAINTAIN BARRICADES, DELINEATORS OR
OTHER TRAFFIC CONTROL AT ALL TIMES.
7. CONTROL POINTS OR "CAT -TRACKING" ARE TO BE APPROVED BY THE CITY TRAFFIC ENGINEER TWO
WORKING DAYS PRIOR TO PAVEMENT STRIPING APPLICATION.
8. CROSSWALKS SHALL HAVE 10' BETWEEN CENTERLINES OF 12" STRIPES PER CITY STANDARD DETAIL

9. ALL CONFLICTING STRIPING AND MARKINGS ARE TO BE REMOVED BY METHOD AS DESCRIBED IN
SPECIAL PROVISIONS, SECTION E. '
10. SUBSTITUTE TYPE "C" RAISED PAVEMENT MARKERS FOR TYPE "G" MARKERS ON DETAIL 38 ON
HIGHWAYS WITH RAISED MEDIANS.
11. BLUE DOT MARKERS (BLUE R.R.P.M.) ARE TO BE INSTALLED AT ALL FIRE HYDRANT LOCATIONS PER
CITY STANDARD PLAN.
12. PARKING CONTROL SIGNS PER DETAIL "1" TO BE INSTALLED AT 1000 FEET SPACING THROUGH
PROJECT SITE IN THE CITY OF FONTANA.
1. CONCRETE BLOCK SHALL BE SPLIT FACE (COLOR: TAN TO MATCH EXISTING WALL) 8X8X16, HOLLOW
UNITS. CONFORM TO A.S.T.M. C-90, GRADE "N-1" MEDIUM WEIGHT UNITS WITH MAX. LINEAR
SHRINKAGE OF 0.06%.
2. GROUT MIX SHALL BE COMPOSED OF ONE PART PORTLAND CEMENT TO NOT MORE THAN THREE
PARTS SAND AND NOT MORE THAN TWO PARTS PEA GRAVEL WITH "GROUT -AID" ADDED FOR HIGH
LIFT GROUTING ONLY. SPECIAL INSPECTIONS SHALL BE REQUIRED FOR HIGH LIFT GROUTING. MINIMUM
GROUT STRENGTH SHALL BE 2000 PSI IN 28 DAYS.
3. ALL REINFORCING SHALL HAVE A MINIMUM COVERAGE OF 1/2" GROUT.
4. ALL MASONRY SHALL BE REINFORCED GROUTED MASONRY. GROUT ONLY CELLS CONTAINING
REINFORCING, UNLESS NOTED OTHERWISE.
5. REINFORCING SHALL BE SECURELY HELD AND BRACED IN PLACE TO PREVENT MOVEMENT WHILE
PLACING MASONRY.
6. ALL CEMENT SHALL CONFORM TO A.S.T.M. C-150, TYPE I OR II, LOW ALKALI
7. FINE AND COARSE AGGREGATE SHALL CONFORM TO A.S.T.M. C-33 FOR STANDARDI WEIGHT
CONCRETE.
8. ALL AGGREGATE SHALL BE COMPARABLE TO "SAN GABRIEL VALLEY" AGGREGATE. THE SHRINKAGE
SHALL BE AS PER A.S.T.M. C-33 WITH THE AVERAGE DRYING SHRINKAGE AT 28 -DAYS OF DRYING
NOT EXCEEDING 0.04X
9. ALL CONCRETE SHALL HAVE MINIMUM COMPRESSIVE STRENGTH OF 2500 PSI AT 28 -DAYS; UNLESS
NOTED OTHERWISE. THE WATER TO CEMENT RATIO SHALL NOT EXCEED 6 1/2 -GAL PER (SACK OF
CEMENT. MINIMUM OF 5 SACKS OF CEMENT PER CUBIC YARD. MAX. SLUMP = 4"VI
