1 . ELEVATIONS ARE IN FEET ABOVE U.S.C. & G.C. MEAN SEA LEVEL DATUM OF 1929
2. NO CONCRETE SHALL BE PLACED UNTIL THE FORMS AND REINFORCING STEEL HAVE
BEEN SET, INSPECTED AND APPROVED.
3. ALL CONCRETE SHALL BE PORTLAND CEMENT CONCRETE WITH AN ULTIMATE 28 -DAY
COMPRESSIVE STRENGTH OF 3250 P.S.I. UNLESS DIFFERENTLY SPECIFIED.
4. ALL EXPOSED EDGES SHALL BE FINISHED WITH A 3/4" CHAMFER.
5. ALL STEEL ADJACENT TO FACE OF CONCRETE SHALL HAVE A 2 1/2" CLEARANCE
UNLESS OTHERWISE SPECIFIED.
6. REINFORCEMENT SHALL BE DEFORMED BARS OF INTERMEDIATE GRADE STEEL AS PER
A.S.T.M. A-615 GRADE 60.
7. ALL BAR BENDS AND HOOKS SHALL CONFORM TO THE AMERICAN CONCRETE INSTITUTE
"MANUAL OF STANDARD PRACTICE".
8. DIMENSIONS FROM FACE OF CONCRETE TO STEEL ARE TO CENTER LINE OF STEEL
UNLESS OTHERWISE NOTED.
9. ALL BACKFILLS AND FILLS TO BE USED AS SUBGRADES SHALL BE COMPACTED TO A
RELATIVE DENSITY OF 90% UNLESS OTHERWISE SPECIFIED. TOP 12" OF SUBGRADE TO
BE 95%. ANY A.B. THICKNESS SHALL BE CONSIDERED AS INCLUDED IN THE 12" DIMENSION.
10. ALL STEEL THAT IS TO BE CONTINUOUS SHALL HAVE A MINIMUM LAP OF 30 BAR
DIAMETERS OR 18" WHICHEVER IS GREATER.
11. PIPE SHALL BE EMBEDDED 5" INTO ALL STRUCTURES INCLUDING INLET AND
HEADWALLS, UNLESS OTHERWISE SPECIFIED.
12. ALL BACKFILL AND FILL AROUND CLOSED CONDUIT IN STREET RIGHT-OF-WAY SHALL
BE BROUGHT UP TO SUBGRADE OF THE ROAD. THE PUBLIC WORKS INSPECTOR
AND THE SOILS ENGINEER SHALL INSPECT ALL BACKFILL AND FILL ABOVE
AFOREMENTIONED LIMIT. APPLICATIONS FOR AN ENGINEER OR INSPECTOR IN CONNECTION
WITH THE WORK SHALL BE MADE BY THE CONTRACTOR AT LEAST 1 WORKING DAY
BEFORE HIS SERVICE IS REQUIRED.
13. ALL REINFORCED CONCRETE PIPE SHALL BE BEDDED IN ACCORDANCE WITH LOS
ANGELES COUNTY FLOOD CONTROL DISTRICT STD. DWG. NO. 2-D177, CASE III.
(SEE BELOW).
14. THIS STORM DRAIN WILL NOT BE ACCEPTED FOR MAINTENANCE UNTIL THE STREETS
HAVE BEEN PAVED, MANHOLES BROUGHT TO GRADE, AND THE SYSTEM IS CLEANED
TO THE SATISFACTION OF THE CITY OF FONTANA AND COUNTY OF SAN BERNARDINO
ENGINEERING DIVISIONS.
15. THE CONTRACTOR ATTENTION IS DIRECTED TO SECTION 7-10.4.1 OF THE STD.
SPECIFICATIONS FOR PUBLIC WORK CONSTRUCTION IN REGARD TO SAFETY ORDERS.
16. THE CONTRACTOR SHALL CONFORM TO THE "MINIMUM PUBLIC SAFETY REQUIREMENTS"
AS SHOWN ON LOS ANGELES COUNTY ENGINEER STD. 5-2.
17. A PERMIT SHALL BE OBTAINED FROM THE CITY OF FONTANA AND COUNTY OF SAN
BERNARDINO PRIOR TO COMMENCING ANY CONSTRUCTION WORK UNDER THIS CONTRACT.
CONTACT THE COUNTY OF SAN BERNARDINO AND THE CITY BY TELEPHONE AT (909) 350-7610
TO OBTAIN INSPECTION PERMIT. CALTRANS PERMIT NO. 08 -99 -N -MC -1464 TO CITY HAS BEEN
OBTAINED FOR CONNECTION TO CALTRANS CHANNEL. CONTRACTOR TO APPLY FOR DUAL PERMIT.
18. THE CONTRACTOR SHALL PROVIDE FOR CONTRIBUTARY DRAINAGE AT ALL TIMES UNTIL
THIS STORM DRAIN SYSTEM IS OPERABLE.
19. THE CONTRACTOR SHALL NOTIFY THE CITY OF FONTANA AT (909) 350-7610 AT
LEAST 24 HOURS BEFORE BEGINING CONSTRUCTION.
20. PRIOR TO START OF ANY CONSTRUCTION, THE CONTRACTOR SHALL OBTAIN A PU13LIC
WORKS CONSTRUCTION PERMIT FROM THE CITY OF FONTANA ENGINEERING DIVISION
AND THE COUNTY OF SAN BERNARDINO.
21. THE PRIVATE ENGINEER SIGNING THESE PLANS IS RESPONSIBLE FOR ASSURING THE
ACCURACY AND ACCEPTABILITY OF THE WORK HEREON, IN THE EVENT OF
DISCREPANCIES ARISING DURING CONSTRUCTION, THE PRIVATE ENGINEER SHALL BE
RESPONSIBLE FOR DETERMINING AN ACCEPTABLE SOLUTION AND REVISING THE
PLANS FOR APPROVAL BY THE CITY.
22. ALL STATE AND LOCAL TRENCH SAFETY RULES WILL BE RIGIDLY ENFORCED.
23. CONTRACTOR SHALL FURNISH A LETTER OF CERTIFICATION FROM PIPE MANUFACTURER
AS TO TEST RESULTS OF PIPE INSTALLED. (MIN. D -LOAD SHALL BE 1350-D).
24. R.C.P. SHALL BE MANUFACTURED AND INSPECTED IN ACCORDANCE WITH SECTION
207-2.7, 2.8 & 2.9 OF THE STD. SPECS. FOR PUBLIC WORKS.
25. ALL RESURFACING, CURBS, GUTTERS, SIDEWALKS, DRIVEWAYS AND ALL OTHER
EXISTING IMPROVEMENTS TO BE RECONSTRUCTED SHALL BE CONSTRUCTED AT THE
SAME ELEVATION AND LOCATION AS THE EXISTING IMPROVEMENTS, UNLESS
OTHERWISE SHOWN ON THE PLANS.
26. ALL PIPES SHALL BE CERTIFIED BY MANUFACTURER AND MARKED WITH D -LOAD,
DATE AND TEE'S LOCATIONS, MARKED FOR PUBLIC WORKS INSPECTOR ACCEPTANCE.
27. ALL PIPE WITH VELOCITIES EXCEEDING 20 F.P.S. WILL HAVE 1/2" THICKENED
INVERTS. (SPECIAL COVER PIPE).
28. PIPES TO BE PLUGGED W/BRICK AND MORTAR SEE DETAIL SHEET 2
29. THE CONSTRUCTION OF ALL PUBLIC IMPROVEMENTS SHALL CONFORM TO THESE
PLANS AND TO THE REQUIREMENTS OF THE STANDARD SPECIFICATIONS FOR
PUBLIC WORKS CONSTRUCTION (GREEN BOOK) AND THE COUNTY OF SAN BERNARDINO.
30. CONSTRUCTION PERMITS SHALL BE OBTAINED FROM FONTANA ENGINEERING DIVISION,
COUNTY OF SAN BERNARDINO AND CALTRANS PRIOR TO START OF ANY
WORK WITHIN THE RESPECTIVE JURISDICTIONAL LIMITS.
31. STATIONING REFERS TO THE CENTERLINES OF STORM DRAINS EXCEPT WHERE
OTHERWISE NOTED.
32. ALL EXPOSED CONCRETE SURFACES SHALL CONFORM IN GRADE, COLOR AND
FINISH TO ALL ADJOINING CURBS AND SIDEWALKS.
33. UNDERGROUND SERVICE ALERT SHALL BE NOTIFIED 2 WORKING DAYS PRIOR
TO START OF WORK (1-800-227-2600)
34. ALL UNDERGROUND WORK SHALL BE COMPLETED PRIOR TO PAVING OF STREETS.
35. EXISTING UNDERGROUND UTILITIES ARE AS PER THE AVAILABLE RECORDS.
THE CONTRACTOR SHALL BE RESPONSIBLE FOR VERIFYING THE ACTUAL
LOCATION AND ELEVATION IN THE FIELD.
36. THE WALLS AND FACES OF ALL EXCAVATIONS GRATER THAN FIVE (5) FEET
IN DEPTH SHALL BE EFFECTIVELY GUARDED BY A SHORING SYSTEM, SLOPING
OF THE GROUND OR OTHER EQUIVALENT MEANS. TRENCHES LESS THAN FIVE
(5) FEET IN DEPTH SHALL ALSO BE GUARDED WHEN EXAMINATION INDICATES
HAZARDOUS GROUND MOVEMENT MAY BE EXPECTED.
37. THE CONTRACTOR'S SHALL ALSO OBTAIN A PERMIT TO PERFORM EXCAVATION
OR TRENCH WORK AS DESCRIBED IN NOTE 18 ABOVE FROM CAL/OSHA.
38. ALL MANHOLES SHALL BE CONSTRUCTED 6" BELOW PAVEMENT GRADE AND
BROUGHT TO FINISH GRADE BY THE PAVING CONTRACTOR AFTER PAVEMENT IS
IN PLACE.
39. IMMEDIATELY FOLLOWING REMOVAL OF EXISTING PAVEMENT OR DIKE OR CURB
AND/OR GUTTER THE CONTRACTOR SHALL DILIGENTLY PURSUE THAT PORTION
OF WORK TO COMPLETION.
40. ALL C.M.P. INSTALLATIONS AND CONNECTIONS SHALL BE MADE PER
MANUFACTURER'S SUGGESTED METHOD UNLESS OTHERWISE SPECIFIED ON THE
PLANS.
41. IF DURING CONSTRUCTION, GROUND WATER IS ENCOUNTERED, A SYSTEM
APPROVED BY THE CITY ENGINEER SHALL BE INSTALLED TO DEWATER SAID
AREA AT THE DIRECTION OF THE SOILS ENGINEER.
42. NO TRENCH BACKFILL SHALL TAKE PLACE WITHOUT PRIOR APPROVAL OF THE
CITY'S INSPECTOR.
43. ALL EXPOSED METAL SURFACES SHALL BE GALVANIZED IN ACCORDANCE WITH
THE CITY STANDARD SPECIFICATIONS.
44. AS -BUILT PLANS SHALL BE PROVIDED TO THE CITY BY THE CONTRACTOR.
45. ALL POTHOLING SHALL BE PREAPPROVED IN WRITING BY THE CITY ENGINEER
PRIOR TO COMMENCEMENT OF ANY WORK.
46. THE CONTRACTOR SHALL BE RESPONSIBLE FOR PROVIDING AN EFFECTIVE MEANS
OF DUST CONTROL WHICH SHALL INCLUDE PROVISIONS FOR ADEQUATE WATERING
DURING THE PROCESS AND PROVISIONS FOR CONTINUANCE OF DUST CONTROL
UNTIL SUCH TIME THAT THE GRADED SURFACE PRESENTS EFFICIENT PROTECTIVE
COVER AGAINST WIND AND WATER EROSION THAT SPECIAL DUST CONTROL MEASURES
ARE NO LONGER NECESSARY.

47. LOCATIONS OF FIELD DENSITY TEST SHALL BE DETERMINED BY THE SOILS ENGINEER
OR APPROVED TESTING AGENY AND SHALL BE SUFFICIENT IN BOTH HORIZONTAL AND
VERTICAL PLACEMENT TO PROVIDE REPRESENTATIVE TESTING OF ALL FILL PLACED.
TESTING IN AREAS OF A CRITICAL NATURE OF SPECIAL EMPHASIS SHALL BE IN
ADDITION TO THE NORMAL REPRESENTATIVE SAMPLINGS.
48. ALL TRAVELED WAYS MUST BE CLEANED DAILY OF DIRT, MUD AND DEBRIS DEPOSITED
ON THEM AS A RESULT OF THE GRADING OPERATION. CLEANING IS TO BE DONE TO
THE SATISFACTION OF THE CITY ENGINEER.
49. ALL CONSTRUCTION AREAS SHALL BE PROPERLY POSTED AND LIGHTED IN CONFORMANCE
WITH THE STATE MANUAL OF WARNING SIGN, LIGHTS AND DEVICES FOR USE IN THE
PERFORMANCE OF WORK UPON HIGHWAYS, IN ORDER TO ELIMINATE ANY HAZARDS.
50. THE CONTRACTOR SHALL NOT DISTURB EXISTING SURVEY MONUMENTS OR BENCHMARKS
NOTED IN THE SPECIFICATIONS OR ON THE PLANS OR FOUND DURING CONSTRUCTION
WITHOUT PRIOR NOTIFICATION TO THE CITY ENGINEER. MONUMENT REPLACEMENT SHALL
BE DONE BY A REGISTERED CIVIL ENGINEER WITH RCE NUMBER 33965 OR LOWER OR
A LICENSED LAND SURVEYOR ONLY.
THE CONTRACTOR SHALL NOTIFY THE CITY ENGINEER ONE (1) WEEK PRIOR TO
CONSTRUCTION SO THAT TIES TO MONUMENTS CAN BE ESTABLISHED.
NOTICE TO CONTRACTORS
1. CONTRACTOR SHALL DEFEND, INDEMNIFY AND HOLD HARMLESS AGENCY
AND THE CITY, THEIR ENGINEERS ( HEREINAFTER ASL CONSULTING ENGINEERS),
THEIR OFFICERS, OFFICIALS, AGENTS, EMPLOYEES, AND CONTRACTORS
FROM AND AGAINST ALL CLAIMS, DAMAGES, LOSSES AND EXPENSES
INCLUDING ATTORNEY'S FEES, ARISING OUT OF OR RESULTING FROM
PERFORMANCE OF WORK UNDER THIS CONTRACT AND WHICH ARE ATTRIBUTABLE
TO BODILY INJURY, SICKNESS, DISEASE OR DEATH, OR TO INJURY TO DESTRUCTION
OF PROPERTY, INCLUDING THE LOSS OF USE RESULTING THEREFROM, CAUSED -1N WHOLE
OR IN PART BY ANY ACT OR OMISSION OF THE CONTRACTOR OR ANYONE DIRECTLY
OR INDIRECTLY EMPLOYED OR ENGAGED BY IT OR FOR WHOSE ACTS IT
MAY BE LIABLE.
2. WITHOUT LIMITING THE GENERALITY OF FOREGOING NOTE NUMBER ONE,
CONTRACTOR SPECIFICALLY AGREES TO INDEMNIFY AND HOLD HARMLESS
AGENCY AND THE CITY, THEIR ENGINEERS (HEREINAFTER ASL CONSULTING ENGINEERS),
THEIR OFFICERS, OFFICIALS, AGENTS, EMPLOYEES, AND CONTRACTORS
FROM AND AGAINST ALL CLAIMS, DAMAGES, LOSSES, PENALTIES, FINES
AND EXPENSES(INCLUDING ATTORNEY'S FEES AND LITIGATION COSTS)
ARISING OUT OF OR IN ANY WAY RESULTING FROM CONTRACTOR'S FAILURE TO
PERFORM THE WORK REQUIRED BY THIS CONTRACT AND APPLICABLE PROVISIONS OF
FEDERAL AND STATE LAW. SUCH INDEMNIFICATION SHALL EXTEND TO ALL CLAIMS,
DEMANDS, ACTIONS, OR LIABILITY FOR INJURIES, DEATH OR DAMAGES
OCCURING AFTER COMPLETION OF THE PROJECT, AS WELL AS DURING THE
WORK'S PROGRESS. CONTRACTOR FURTHER AGREES THAT IT SHALL
ACCOMPLISH THE ABOVE AT ITS OWN COST, EXPENSE AND RISK EXCLUSIVE
OF AND REGARDLESS OF ANY APPLICABLE INSURANCE POLICY OR POSITION
TAKEN BY ANY INSURANCE COMPANY REGARDING COVERAGE.
3. THE LOCATION OF EXISTING UNDERGROUND UTILITIES ARE SHOWN IN AN
APPROXIMATE WAY ONLY. THE CONTRACTOR SHALL DETERMINE THE EXACT
LOCATION OF ALL EXISTING UTILITIES BEFORE COMMENCING WORK. HE AGREES
TO BE FULLY RESPONSIBLE FOR ANY AND ALL DAMAGES WHICH MIGHT BE
OCCASIONED BY HIS FAILURE TO EXACTLY LOCATE AND PRESERVE ANY AND
ALL UNDERGROUND UTILITIES.
4. AB 73 REQUIRES EVERY PERSON PLANNING TO CONDUCT EXCAVATION TO
NOTIFY A REGIONAL NOTIFICATION CENTER AT LEAST TWO DAYS PRIOR TO
COMMENCING WORK. A VERIFICATION NUMBER WILL BE GIVEN UPON SUCH
NOTIFICATION WITHOUT WHICH ANY PERMIT OR DIRECTIVE GIVING PERMISSION
TO EXCAVATE SHALL BE INVALID. VERIFICATION NUMBERS EXPIRE WITHIN
14 DAYS. FOR BEST COMMUNICATION, PERSONS PERFORMING EXCAVATION
SHOULD MEET THE UTILITY COMPANIES AT THE JOB SITE. FOR MORE
