Attached for filing by the City Clerk's office= is the original
recorded Notice of Completion for the Construction of the Locust
Avenue Sewer Extension from 110 Feet South of Foothill Boulevard to
170 Feet North of Foothill Boulevard. The contractor on the
project was S.J. Burkhart, Inc. The project was accepted as
complete by the Council on June 18, 1996.

NOTICE OF COMPLETION
Pursuant to the provisions of Section 3086 of the Civil Code of the State of California and
authorization of the City Council of the City of Fontana, NOTICE IS HEREBY GIVEN that work on that certain
project known as Locust Avenue Sewer Extension for the City of Fontana was completed and accepted by the
City of Fontana on the 18th day of lune. 1996,.
The said work was performed by S.J. BurkhardL Inc. of 6157 Marlatt Street, Mira Loma. CA
91752 in accordance with the terms of written agreement dated the 1st day of April, 1996.

RE: Locust Avenue Sewer Extension Project from 110' South of
Foothill Blvd. To 170' North of Foothill Blvd. (SB-72-94)
Dear Mr. Stuart:
This letter will serve as the "Notice to Proceed" on the above referenced project. As per the
construction contract all contract work shall be completed in twenty-fiw,· (25) working days after
the "Notice to Proceed".
All traffic control must be in place and checked by Caltrans and City Inspection Staff prior to
commencing any excavation related work on this project.
