A_=. GENERAL DISCUSSION OF CONTRACTUAL, LEGAL AND PERMIT ASPECTS:
STATUS OF CONTRACT: In process of gettin~ executed by the CitV
Officials.
INITIATING CONSTRUCTION:"NOTICE TO PROCEED" Will be issued
after contract is fully executed.
~EQUIRED PERMITS: Prior to the start of any work, the
Contractor shall take out the applicable CITY, Caltrans and
County permits (SP-8, Sec. 7-5].
COMPLETION TIME FOR CO~i'r~ACT:The Contractor has 35 workin~
days after "Notice to Proceed" is issued, excluding Holidays.
LIQUIDATED DAMAGES: The City and Contractor agree that if the
work is not completed within the period provided in the
General Conditions that the sum of $500 for each and every
Calendar day of such delay shall be charged. (SP-5, Sec. 6-9)
OTHER REQUIREMENTS OF CONTRACT DOCUMENTS:(To include City of
Fontana Business License and all Appropriate State of
California Required Licenses).
Contractor and all Sub-Contractors shall be required to
provide a copy of their City of Fontana Business License to
the Pro~ect Manager.
B_~. CONTRACTORS SCHEDULE:
CONTRACTORS SCHEDULE:The Contractor shall submit an updated
Construction Schedule to the ProgectManaqer'on a month to
month basis. Schedule shall be supported by written
statements from material suppliers. (SD-4,SeC.6-1)
SUBCONTRACTORS:No chan~e in subcontractors is allowed
Drior to the written approval from the City of Fontana.
WORKING HOURS:Working hours shall be confined to the hours
between 7:00 a.m. and 6:00 p.m., Monday through Friday.
(SP-5, Sec. 6-7.2)
C. PAYMENTS
PROGRESS PAYMENTS: ProUress PaYments shall be submitted in the
format required bv the City fSee Sample Provided). Any
progress payment submitted differently than. the format
rec~lired by the City, shall be returned for correction. The
date of submittal will be counted from the date the final
corrected CODV is received in Aocounts Pavable. To simDlifV
the Drocess for progress payments they shall be submitted to
the Engineering Division before submitting to Accounts Payable
for Daymerit.
10% RETENTION MONIES:PUrsuant to contract documents.
CHANGE ORDERS:NO extra 'work will be Dreformed without
aDDroved chan~e order from City Engineer.
CERTIFIED PAYROLLS: Certified Payroll Records shall be
submitted to the CITY by the tenth day of each month.
Contractor will be responsible for Certified Payroll for his
Sub-Contractors. Prouress payments will be withheld pending
receipt Of any outstanding reports [SP-12, Sec. 7-15).
D. RIGHT OF WAY AND UTILITIES:
NOTIFICATION OF AGENCIES:The County of San Bernardino, Cal -
trans, and any other related A~encv shall be notified 48 Hrs.
in advance "Prior" to s'tartworkin~ within their right of waV
or as required bv the sDecific agency.
COORDINATION WIT~OT~ER AGENCIES: The Contractor needs to make
arrangements for Citv, County and Caltrans inspections.
[SP-8. Sec. 7-5).
RIGHT OF WAY REQUIREMEntS: Citv will acquire all rights of
way, easements and rights of entry as required for this
proiect. The Contractor shall verify that the acquisition(s)
is comDleted prior to beginnin~ any work outside the public
right of way. (SP-2. Sec. 2-8).
~0TIFICATION OF UTILITIES:Call "DIG ALERT" and provide Citv
with their confirmation n,,mher prior to start of work.
Notify non-member utilitV companies separatelY.
COORDINATION WIT~ UTILITIES: Must coordinate with all
utilities to make sure that their will be no interruDtion of
services to customers.
E~SEMENTS REQUIREMENTS:~(UTILITIES) NONE. Fontana Water
Company has notified City that the 2 inch water main south of
street center line has been abanodoned in place.
E. ENGINEERING/INSPECTION AND SURVEYING
STAKING: The EnGineer will provide surveying and construction
staking required for the construction of this project as
determined bv the Engineer (SP-2, Sec. 2-9.3). All requests
shall be made through the City Inspector.
RECORD DRAWINGS:Contractor to provide red-lined blueprint
markups of all changes to plans at the end of the contract
work
MATERIALS AND SOILS TESTINGREQUIREMENTS: Field density tests
will be made bv the Engineer during the course of construction
at the expense of the City (SP-13, Sec. 211-2.2). Only City
Inspector can call for testing services.
F~ CONSTRUCTION
I~AFFIC CONTMOL: Special attention is called out in Section
7~10.0. The Contractor's operations shall cause no
unnecessary inconvenience. The access riqhts of the public ~
shall be considered at all times, Unless otherwise authorized
traffic shall be permitted to Dass through the work.
(SP-9, Sec. 7-10.1).
PROTECTION OF EXISTING FACILITIES: Contractor shall protect
in place said utility and provide all labor, materials, tools
and e~uipment necessar~ for shoring, bracing or other support
and backfill required by utility owner of City and no other
additional compensation will be allowed (SP-8, Sec. 7-9).
DUST CONTROLS: Contractor shall be responsible to maintain
dust control at all times including Holidays and Weekends.
STORAGE OF MATERIALAND EQUIPMENT: No more than 1,000 linear
feet of pipeline shall be stockpiled on site, regardless of
size. The Contractor may at his own expense, maintain and
operate a work and storage area outside the public riGht-of-
way. In such case the contractor shall submit to CITY written
authorization from the owner (SP-10, Sec. 7-10.2].
JOB SAFETY: Shall be maintained at all times in accordance
with all applicable laws.
JOB SITE SECURITY: Contractor responsibile at all times.

0UESTIONS: The connection of existinq sewer to Washinqton
Drive will be made only after the sewer main is complete
and connected to main line in Cypress Avenue and video

FINAL INSPECTION REPORT: Proiect will not be accepted for
operation and maintenance until all Dunch list items are
corrected, Landscape Maintenance Period completed and is to
the satisfaction of the City Inspector.
FINAL ACCEPTANCE BY CITY COUNCIL: The followinq conditions
shall be fulfilled:
1.) city Landscape Technician shall verify that Landscape
Maintenance Period has been completed to the City's
satisfaCation.
2.) Proiect accountinq shall be fullv aqreed uDon bV the
Contractor and the City. There shall be no disputed
items.
Proiect will then be scheduled for the City Council meeting
for Final Acceptance.

Pre-Construction Meeting
Dear Mr. Burkhardt:
The City of Fontana will be conducting a pre-construction meeting
for tile above referenced project.
The pre-construction meeting is scheduled for Wednesday, MaV 24,
1995 at 9:00 am at the City of Fontana City Hall in the Design
Advisorv Board Meetinq Room (D.A.B.) located at 8353 Sierra Avenue,
Fontana, CA. 92335. The project manager, job superintendent and
subcontractors are required to attend this meeting.
You are required to submit 'the following items at the ~eeting:
1.) Construction Schedule
2.) Emergency Information Form (Copy Enclosed)
3.) Names and titles of persons entitled to authorize and
sign change orders.
4.) Copy of City of Fontana Business License for contractor
and subcontractors as required per Fontana Municipal
Code, Sec. 15-36.
!5.) Copies of Permits from other Agencies.
"Notice to Proceed" will not be issued until these conditions are
met.

Emergency Information
The names, addresses and telephone numbers of the contractor and sub-contractors or his representatives,
shall be filed with the City Engineer and the Police Department prior to beginning work.
The contractor shall provide adequate detours to the satisfaction of the City Traffic Engineer. A detailed
plan of such detours shall be submitted to the; City Traffic Engineer at least five (5) days prior to the time
the contractor wishes to close off the street or in any way restrict the traffic flow. In actual street
closures, the submitted plan shall indicate the mute of the detour, the character of the temporary
pavement where required, and a layout of the locations of signs and lighting. In all other cases, the
contractor shall provide adequate traffic flow in both directions. If traffic cannot be maintained the
contractor shall provide flagmen whose sole; duties consist of directing traffic through and around the
work. In all cases, the approval of a suitable ]plan shall be granted to the contractor prior to the beginning
of construction.
Failure or refusal by the contractor to comply with the above mentioned requirements shall be sufficient
cause for the City Engineer to order the work done by city forces and the cost therein to be borne by the
contractor.
