SUBJECT REF: BASE LINE AVENUE AND ALMERIA AVENUE TRUNK SEWER
Dear Mr. Molinos:
As per your meeting with Milt Madole and subsequent communi-
cation, we are pleased to submit for your consideration a
proposal for certain office and field engineering for the above
referenced project. Following are Exhibits A, B, and C, which
along with this letter constitutes Madole and Associates, Inc..'s
proposal. We would propose to perform the work for a fee of
$14,036.00 in accordance with Exhibit B. Work uncompleted as
of August 1, 1992, would be escalated in accordance with
industry wide increases at that time.
As has been our practice in the past, invoices would be sent
every four (4) weeks for that portion of the work performed
during the immediate past four (4) week period. Unless we hear
contrary within fifteen (15) days of date of invoice, we will
assume that the invoice is acceptable and will be put in line
for payment and that payment will be made no later than thirty
(30) days after date of invoice. This proposal will remain
valid until September 23, 1991 when it will become null and void
if not accepted before that time.
We appreciate the opportunity to submit this proposal to you.
Should. you wish us to proceed with the work, a signed Purchase
Order is all we need to commence.

EXHIBIT A
ENGINEER will complete the following tasks:
OFFICE:
1. Prepare a 40 scale temporary detour road plan. The plans
will be drawn on standard 24" x 36" mylar sheets with City
of Fontana title block information. A title sheet will be
included. The plan will consist of plan view data only (no
profile). It will show the precise alignment and
construction of the proposed temporary detour roadway.
Included in the data will be existing spot elevations at
approximately 50 feet spacing, with shots 50 feet north and
50 feet south of the centerline. Vertical grades will be
shown along the centerline only on the plan view. The
plans will include pertinent data at the join points at
Base Line Avenue. Typical sections, quantities and
constructions notes will be shown.
2. Prepare easement deeds for the various parcels affected by
the temporary detour road. Included in the scope of this
work will be all communication with Chicago Title Company,
review of present title description, vesting, deed
preparation including legal description. Not included in
this work will be work necessary to obtain the owners
signature. Also not included will be the cost of
preliminary title reports or title policies.
3. Prepare an easement deed for the "Almeria Avenue
alignment". This deed will include a strip of land, 60
feet, extending from Base Line Avenue to the easterly
boundary line of Tract No. 12314 at its northerly terminus.
4. Minor modifications to the specifications as identified by
the City of Fontana as well as minor modifications to the
Base Line Avenue Trunk Sewer Plans. (Note - "minor
modifications" are defined as those corrections and/or
revisions which would require less than twelve (12) hours
total to complete.)
FIELD:
1. All field survey work required for determination of
existing ground elevations along the temporary detour road.
Elevations will be at approximately 50 foot intervals and
will extend 50 feet north and 50 feet south of the proposed
road centerline. Also included will be tree location,
fence location, utility -pole location and other topographic
features crossing the temporary detour road.

STANDARD BILLING RATES FOR TIME AND MATERIALS
EXTRA WORK AND OUTSIDE SERVICES
The CITY OF FONTANA agrees to pay ENGINEER as compensation for
all authorized or additional work heretofore stated, at the
hourly rates set forth below. All such extra or additional work
must be authorized by CITY OF FONTANA in writing to ENGINEER
prior to commencement of such extra work by
authorized agent for CITY OF FONTANA.

DESIGNER - $ 82.00 PER HOUR
DRAFTER - $ 56.00 PER HOUR

TWO MAN SURVEY CREW - $ 148.00 PER HOUR
OUTSIDE SERVICES
DEVELOPER shall pay the costs of all agency fees, permits, bond
premiums, title company charges, delivery charges, blueprints
and reproductions and all other outside charges and expenses.
