NOTICE OF COMPLETION
Pursuant to the provisions of Section 3086 of the Civil Code of the State
of California and authorization of the City Council of the City of Fontana,
NOTICE IS HEREBY GIVEN that work on that certain project known as
Construction of the Baseline Avenue/Almeria Avenue Trunk Sewer System
Project No. SB-45-93 , for the City of Fontana was completed and accepted by the
City of Fontana on the 7th day of September , 1921.
The said work was performed by Albert W. Davies. Inc.
of 8737 Helms Avenue. Rancho Cucamonga. CA in accordance with the terms of
written agreement dated the 2nd day of March , 1993.

INSTRUCTIONS O RESIDENT ENGINEER
1. When recording nonworking days due to weather, state the
reason why the day was unworkable when the weather description
itself does not adequately descrive conditions. For example,
"Clear -wet grade" to describe conditions when the weather is
clear, but the grade is too wet to work. • Do not list days
merely as "Unworkable."
2. Enter days on which no productive work haslbeen.performed on
the controlling operation(s) for reasons other than weather.
3. List numbers of contract change orders providing for time
extensions.
4. Do not include nonworking days which occur after expiration of
the Extended DATE OF COMPLETION. On contracts that are
overtime, the total under WORKING DAYS shall not be greater
than the total of REVISED WORKING DAYS, (Line 8). After
approved total of working days has been reached, continue
recording working and nonworking days but do not add into the
totals. Make statement under REMARKS that WORKING and
NONWORKING days are shown for record only since the contract
time has elapsed.

Re: Temporary Traffic Control and Road Closure for the Baseline
Avenue/Almeria Avenue Sewer Improvement Project.
Dear Mr. Davies:
The subject road closures and detours of Baseline Avenue from Cherry Avenue
east to Citrus Avenue, for the dates of June 8, 1993 through August 10,
1993 has been approved by the City with the following stipulations:
1. Notification must be given to the emergency service agencies --
Police Department, County Sheriff's Department, Fire Department,
Ambulance Company, Transportation Superintendent of Fontana
Unified School District, Com Center and to the affected property
owners. A Street Closure Notification List has been enclosed.
2. There can be no other closures or detours which conflict with
this traffic control.
3. The starting date for the closures and detours cannot be sooner
than June 8, 1993. Extension of this road closure and detour may
be authorized only upon expressed approval by the City Traffic
Engineer.
4. Access for affected residents, property owners and business in
the affected area, shall be maintained at all times.
5. You must adhere to the traffic control plan which you have
provided and which has been approved by the City Traffic
Engineer.
6. You shall have approval from the Caltrans inspector for signs
placed on Foothill Boulevard (Route 66) before the City will give
final approval of the traffic control and detour.

INSTRUCTIONS TO RESIDENT ENGINEER
1. When recording nonworking days due to weather, state the
reason why the day was unworkable when the weather description
itself does not adequately descrive conditions. For example,
"Clear -wet grade" to describe conditions when the weather is
clear, but the grade is too wet to work. Do not list days
merely as "Unworkable."
2. Enter days on which no productive work has been performed on
the controlling operation(s) for reasons other than weather.
3. List numbers of contract change orders providing for time
extensions.
4. Do not include nonworking days which occur after expiration of
the Extended DATE OF COMPLETION. On contracts that are
overtime, the total under WORKING DAYS shall not be greater
than the total of REVISED WORKING DAYS, (Line 8). After
approved total of working days has been reached, continue
recording working and nonworking days but do not add into the
totals. Make statement under REMARKS that WORKING and
NONWORKING days are shown for record only since the contract
time has elapsed.
5. From calendar issued by Headquarters with working days

7-2.2 Laws. (Add the following):
The Contractor, and all subcontractors, suppliers and vendors shall comply with
all CITY, State and Federal orders regarding affirmative action to ensure equal
employment opportunities and fair employment practices. Failure to file any
report due under said orders will result in suspension of periodic progress
payments.
The Contractor shall ensure unlimited access to the job site for all equal
employment opportunity compliance officers.
7-3 LIABILITY INSURANCE. (Replace the entire Subsection with the
following):
7-3.1 Indemnification.
The Contractor shall indemnify and save harmless the CITY OF FONTANA, the State;
the County and any incorporated cities from all claims or suits for damages
arising from his prosecution of the Contract work, as more fully described in
Subsection 7-3.2 "Contractor's Liability."
7-3.2 Insurance Requirements.
The insurance afforded by this policy shall not be canceled, suspended, or
modified or renewal of such a policy declined unless notice is mailed, by
certified mail return receipt requested to the City at least 45 days prior to the
effective date of the nonrsnswal, suspension, or modification or at least 60 days
prior to the effective date of cancellation.
The Contractor shall maintain during the life of the contract and the entire
progress of the work, and until sixty (60) days after notice of completion has
been filed a Comprehensive Automobile and General Liability policy. The policy
shall provide for not less than the following amounts:
